چکیده

Although we applaud the eff ort by Koenig and colleagues 1 presented in a recent issue of CHEST (April 2014) to transmit the message that point-of-care ultrasonography may help to avoid unnecessary CT pulmonary angiography (CTPA) in patients with suspected pulmonary embolism (PE), we wish to raise a few concerns regarding the study. We believe the fact that only 12 patients (12.5%) undergoing a CPTA had a PE diagnosed refl ects poor clinical decision-making. Th ere was no mention in the article about either the Wells score or revised Geneva score in this cohort, which may have helped us to understand the real need for a CTPA in these patients. As experienced clinicians, we believe that the patterns discovered by the authors on ultrasonography, namely alveolar consolidation, pleural eff usion, and pulmonary edema, can be detected by a plain chest radiograph most of the time, and alveolar consolidation and pulmonary edema can be suspected from a proper clinical history and chest examination. Th e chest radiograph fi ndings are not presented as well.

Diagnosis of thromboembolic disease: Combined ventilation perfusion lung scan and compression ultrasonography

The clinical management of pulmonary embolism and deep venous thrombosis of the legs are similar and requires prolonged anticoagulation therapy. The standard diagnostic approach in patients suspected of pulmonary embolism is ventilation-perfusion (V/Q) lung scan and compression ultrasonography to detect deep venous thrombosis. This retrospective study analyzed the role of V/Q lung scan an...

Comparison of Image Quality of Low Voltage 64-slice Multidetector CT Angiography with the Standard Condition in Patients Suspected of Pulmonary Embolism

Introduction: Reduction of peak kilovoltage (kV) setting has been a useful approach to d creating radiation dose; however, it may have varied effects on noise and the accuracy of diagnosis. Thus, we compared image quality between low (80 kV) and standard kilovoltage (100 kV) protocols. Material and Methods: This triple blind non-randomized parallel quasi-experimental study was conducted on 140 ...

The Diagnostic Value of End-tidal Carbon Dioxide (EtCO2) and Alveolar Dead Space (AVDS) in Patients with Suspected Pulmonary Thrombo-embolism (PTE)

Introduction: Capnography, is an easy, fast and practical method which its application in the diagnosis of Pulmonary Thromboendarterectomy (PTE) has recently been studied. This study aimed to assess the diagnostic value of end-tidal CO2 (ETCO2) and the alveolar dead space (AVDS) in the diagnosis of patients suspected to PTE who have been referred to the emergency department. Materials and Metho...
